Adam Mobile Apps (“we”, “us”, or “our”) respects your privacy. This Privacy Policy explains how Patient Forms, available via the Google Play Store and Apple App Store, collects, uses, discloses, and protects your information. The app is intended for use by verified AW3 members to manage and record client treatment data using AW3 equipment.

We collect the following types of information from users and/or their clients:

a. Personal Information (User/Practitioner)

Full name
Email address
Profile picture (if uploaded)
Login credentials (encrypted)
Device identifiers (e.g., Android ID, iOS Identifier for Advertisers – if applicable)

b. Customer (Client) Data (Input by the Practitioner)

Customer name (optional)
Treatment details
Before and after images
Notes or remarks entered by practitioner

Note: You (the AW3 member) are solely responsible for obtaining necessary consent from your clients before entering or storing their data.

c. Usage Data & Device Information

App version, device type, operating system
Error logs and crash reports
IP address (used temporarily for connection and security purposes)

3. How We Use Your Information

We use the data collected to:

Provide and maintain core app functionality
Allow secure access and authentication for AW3 members
Store and manage treatment data on your behalf
Perform backup and restoration services
Analyze bugs, crashes, and improve app stability
Share anonymized before/after images (with consent) on our social media platforms and website

4. Legal Basis for Processing (Under GDPR)

Our processing of your data is based on:

Legitimate interest – to provide and support the platform functionality
User consent – for sharing media on social platforms
Legal obligation – in case of regulatory requirements

5. Data Sharing and Disclosure

Practitioner Profile Data: Retained until the account is deleted or you request erasure.
Client Data: Stored for as long as the practitioner maintains it within the app.
We reserve the right to retain anonymized logs for analytical and legal purposes.

We implement strong measures to safeguard your data, including:

SSL encryption in transit
Secure, encrypted cloud storage
Regular backups and server monitoring
Access control and user authentication

However, no method of storage or transmission is 100% secure. Users are advised to regularly back up client records locally via CSV export.

Our web-based portal may use cookies for the following:

User session tracking
Saving preferences
Analytics and performance (non-identifiable)

By using the site, you consent to cookie use.

The mobile app itself does not use cookies, but it may collect anonymized usage data for diagnostics.

Patient Forms is not intended for use by individuals under the age of 18. We do not knowingly collect personal data from children. If you believe that a child has provided us data, please contact us so we can delete it.

12. App Store Compliance

Google Play: We comply with Google’s User Data and Permissions policy.
Apple App Store: We comply with Apple’s App Store Review Guidelines and App Tracking Transparency requirements.
This app does not serve ads or use third-party ad tracking SDKs.

13. Changes to this Privacy Policy

We may update this policy from time to time. All changes will be posted here and, where appropriate, notified to you in-app or via email.
